According to medieval tradition the title was given to 12 men which took part in the fight against the moors in Spain; one of them was the hero of the Roland's song. The Paladins were topic of medieval literature, for example ballads and the tale of King Arthur, as an ideal of knightly politeness/fairness and bravery. (From the Swedish National encyclopedia)
